Checklist for Accepted Students

Here are some requirements that must be met for students who have been accepted into the Class of 2023. Please read this information carefully and note the dates that information is due. Forms must be submitted through our tracking system. Information and login information for our tracking system will be provided during Orientation Week.

Have you done the following?

  • Collect and prepare your Immunization Record?*

    Learn more about our immunization requirements. Download the AAMC Standardized Immunization Form (PDF).
    Note: This is a new form as of Jan. 11, 2016. The AAMC Form is required but does not explain our specific immunization requirements.

    The deadline for submitting the AAMC Standardized Immunization Form and other records to CastleBranch (same account as health record) is June 30. Please begin gathering your immunization data ASAP so that there are no delays. For more information, contact som-admissions@wright.edu.

  • Submitted your Health Insurance Enrollment Form or Waiver Form?

  • Submitted your Life & Disability Insurance Enrollment Form?

  • Sent your official transcripts?

    The state of Ohio requires that you provide us with official transcript(s) from all colleges and/or universities you have attended. If you received a degree, the transcript(s) must have your degree(s) posted. The deadline is July 1. All prerequisite coursework should be completed by July 1 and reflected on the submitted transcript(s). The transcript(s) you release to AMCAS when applying are NOT sent to us. You must request that your official transcripts (with all prerequisites and degrees listed) be sent to us by July 1. Please have your official transcript(s) submitted electronically to som-admissions@wright.edu or mailed to:

  • Completed your Certiphi Background Check?

    In January of the matriculation year, or sooner, AAMC will initiate background checks with Certiphi Screening Inc. for all accepted students. When this occurs, you will receive an email requesting your consent. Once your consent is provided, Certiphi Screening, Inc. will procure a national background check on you. The completed report will then become available to you to review prior to its distribution to your designated medical schools.  You will have ten (10) calendar days from the date you receive your completed background check report to review it before it is made available to your designated participating medical schools. If you do not review the report, the report will be distributed after this period elapses. More information can be found on the AAMC Criminal Background Check webpage.

* Maintenance of health records All medical information will be submitted to CastleBranch by the student. The Boonshoft School of Medicine does not accept any student medical records.
